Job Summary
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team of healthcare professionals. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to geriatric patients in partner facilities.
Responsibilities
- Round and manage clinical care for geriatric patients in partner facilities, including skilled nursing, long-term care, assisted living, independent living, and memory care facilities.
- Travel to facilities to provide medical coverage 4 days/week.
- Build relationships with facility staff, administrators, and nursing directors.
- Engage with the multi-disciplinary care team to address quality of care and patient care needs.
- Complete timely documentation and billing for services provided per Curana policy.
- Respond to daytime calls from the facility when not physically present.
- Assist with the transition to the permanent Curana provider(s).
- Serve as a model and representative of Curana Health at all times.
