About the Role

Amedisys Inc. is seeking a skilled and empathetic medical social worker to join our homecare team.

As a Medical Social Worker, you will play a vital role in assisting patients and families with health-related concerns, providing education on medical interventions, and collaborating with the healthcare team to ensure comprehensive care.

Key Responsibilities:

  • Assist patients and families with financial, social, and emotional challenges related to their health.
  • Develop and implement patient-centered plans of care that address their unique needs and circumstances.
  • Communicate effectively with the healthcare team, including physicians, nurses, and other staff members, to ensure seamless care coordination.
  • Provide education and support to patients and families on managing chronic conditions, navigating the healthcare system, and accessing community resources.
  • Maintain accurate records and reports, ensuring compliance with regulatory requirements and company policies.
  • Collaborate with interdisciplinary teams to identify and mitigate risks to patient health and well-being.

Requirements and Qualifications:

  • Master's or doctoral degree in social work from a Council on Social Work Education-accredited program.
  • At least one year of experience in a healthcare setting, preferably in social work or a related field.
  • Current CPR certification and meeting state and federal requirements for the position.
