We are currently in search of a Registered Nurse II for our Gender Affirming Care Program located in Worcester. This role involves collaborating with a multidisciplinary healthcare team, focusing on delivering high-quality primary medical care to patients of diverse backgrounds, ages, and ethnicities. The nurse will adhere to established nursing protocols, assessing, treating, and educating patients on a range of medical conditions. Additionally, the nurse will provide coordination of care and emotional support to patients' families, working as an integral member of the clinical practice team.

Key Responsibilities:
- Deliver direct patient care within the scope of nursing licensure, including patient assessments, administration of vaccinations, medications, and treatments.
- Provide education on injection techniques and medication management for transgender patients.
- Assist with clinical documentation for medication prior authorizations and pre-surgical coordination as directed by healthcare providers.
- Delegate appropriate patient care tasks to ensure efficient patient flow within the organization.
- Facilitate coordination of patient care outside the organization, including medication management, review of diagnostic results, and follow-up on external services.
- Conduct point-of-care testing as required.
- Document patient interactions in the electronic health record (EHR) in compliance with organizational and nursing standards.
- Adjust patient care strategies based on age and developmental needs.
- Comply with organizational policies and procedures related to nursing practice.
• Triage patients presenting with acute symptoms, either in person or via telephone.
• Request medical interpretation services as needed.
• Assist in training new nursing staff or mentoring students.
- All employees are required to have the latest COVID-19 booster and the annual flu vaccine.
- This position may involve evening, weekend, and/or holiday shifts on a rotating schedule.
Preferred Qualifications:
- Current MA Registered Nurse License or LPN license.
- Basic Life Support (BLS) certification.
- Minimum of 2 years of external nursing experience, with at least 1.5 years at Kennedy Community Health.
- Experience and/or a strong interest in working with transgender individuals.
