Job SummaryWe are seeking a skilled and compassionate Part-time Per Diem Nurse to join our team. As a Home Health Care Professional, you will be responsible for providing high-quality patient care and support in the comfort of their own homes.
Key Responsibilities:- Conduct initial assessments of patients to determine their home care needs
- Develop and implement personalized care plans to meet the unique needs of each patient
- Provide skilled nursing care and support to patients in their homes
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination
- Accurately document patient information and care plans
- Current and unrestricted California RN license
- Current BLS/CPR certification
- Valid California driver's license
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience working with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
