Cleveland Clinic is seeking a compassionate and skilled Hospice Home Health Aide to join our team. As a Hospice Home Health Aide, you will play a vital role in providing patient-centered care to individuals and their families in the comfort of their own homes.
Key Responsibilities- Perform basic patient care activities, including personal care, activities of daily living, and transfers/transport
- Assist with procedures ordered by physician and supervised by an RN, PT, OT, or ST
- Report and document abnormal assessment findings or changes in physical, mental, and emotional conditions to nursing staff
- Maintain a clean and orderly patient environment
- Escalate changes in patient condition to patient's physician
- High school diploma or GED
- Certification and registration with the State of Ohio Nurse's Aide Registry (STNA)
- One year of clinical experience as an aide in an acute, rehab, or skilled nursing setting
- Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage
- Basic Life Support (BLS) certification through the American Heart Association (AHA)
- Hospice and Palliative Care Nurse Assistant (CHPNA) certification
