Job Title: Registered Nurse - Home Health Case Manager

St. Luke's Health System is seeking a skilled Registered Nurse (RN) to join our Home Health team as a Case Manager. This is an exciting opportunity to provide exceptional patient-centered care in the comfort of patients' own homes.

About the Role:
  • As a Home Health Case Manager, you will be responsible for coordinating patient care, developing care plans, and collaborating with healthcare teams to ensure seamless transitions.
  • You will work closely with patients, families, and caregivers to understand their unique needs and preferences.
  • With a focus on patient-centered care, you will prioritize their comfort, safety, and well-being throughout the care process.
Responsibilities:
  • Conduct comprehensive assessments of patients' medical, social, and emotional needs.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Collaborate with healthcare teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care.
  • Coordinate patient care, including scheduling appointments, arranging transportation, and managing medications.
  • Provide education and support to patients and families to promote self-care and independence.
Requirements:
  • Current RN license in Idaho.
  • 2 years of RN experience, preferably in home health or case management.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
