Our comprehensive network of services includes primary care and outpatient facilities, home health care, urgent care centers, three hospitals (1,336 beds), a freestanding emergency department, a Level I trauma center, and a Level III neonatal intensive care unit, as well as regional centers of excellence in heart and vascular care, cancer care, and women's health.
Job DescriptionWe are seeking an experienced Registered Nurse to join our Case Management team as a RN Case Manager - Day Shift. This is a full-time position with a competitive salary range of $90,000 - $120,000 per year, depending on experience. In addition to a generous compensation package, we offer a comprehensive benefits program, including medical, dental, vision, life insurance, tuition assistance, and a 403(b) retirement plan.
The ideal candidate will possess a strong background in medical-surgical nursing, preferably in an acute care hospital setting. A minimum of 2 years of experience in this field is required, with a preference for candidates who have worked in case management or discharge planning roles.
Key Responsibilities- Coordinate patient care and develop effective discharge plans to ensure timely and appropriate transitions throughout the care continuum.
- Collaborate with interdisciplinary teams to identify and address system issues that may hinder patient progression.
- Utilize advanced communication skills to facilitate clear and concise information exchange among healthcare providers, patients, and families.
- Analyze complex patient data to inform informed decision-making and optimize patient outcomes.
