Position Overview: The Stroke Care Nurse Specialist plays a pivotal role in delivering exceptional nursing care tailored to a specific group of stroke patients, integral to the Stroke Services framework.
Key Responsibilities:
- Provide personalized navigation services to patients from the moment they enter the healthcare system until their discharge, specifically for those who have suffered an ischemic or hemorrhagic stroke.
- Act as a vital resource for stroke patients, their families, and healthcare professionals, serving as an educator, quality improvement advocate, and consultant.
- Collaborate closely with departments such as Neurology, Medicine, and Neurosurgery to ensure comprehensive care.
Compliance and Standards: All duties are performed in alignment with applicable laws, regulations, and the philosophy, policies, and standards of Medstar Health.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) is required.
- Experience: A minimum of 3-4 years of professional clinical experience is essential, with 1-2 years of recent experience in neurological care preferred.
- Licenses and Certifications: Current RN License, Basic Cardiac Life Support (BCLS), Advanced Cardiac Life Support (ACLS) are required; certifications such as CNRN or SCRN are preferred.
Knowledge, Skills, and Abilities:
- Strong understanding of nursing theory and practice to effectively assess, plan, implement, and evaluate patient care.
- Proficient in the use of medical equipment and instruments necessary for patient care.
- Ability to maintain effective working relationships with patients, medical staff, and the public.
- Capacity to remain calm and effective in emergency situations.
Quality Improvement: Contribute to achieving departmental goals and adhere to quality and safety standards. Develop and implement evidence-based strategies to enhance stroke care quality, ensuring compliance with relevant certifications.
Clinical Services: Provide professional clinical services in both inpatient and outpatient settings, including:
- Conducting patient assessments and facilitating communication between patients and healthcare providers.
- Identifying and addressing barriers to care.
- Collaborating with the Stroke Coordinator to ensure adherence to established guidelines.
Patient Education: Coordinate educational initiatives for patients and families, assessing their learning styles and health literacy to deliver tailored education throughout their treatment journey.
Community Engagement: Partner with community organizations to promote stroke prevention and care education.
Follow-Up Care: Ensure effective communication with patients and families post-discharge to reinforce the importance of follow-up care.
Research Participation: Identify opportunities for patient involvement in clinical trials and facilitate communication with research coordinators and referring physicians.
Team Collaboration: Participate in meetings and committees, representing the department in community outreach efforts and contributing to multidisciplinary quality improvement teams.
Perform additional duties as assigned that align with the role.
