About the Job

We are seeking an experienced Occupational Therapist to join our team at LHC Group. As a member of our home health care team, you will play a vital role in providing high-quality patient care and supporting individuals in achieving their goals.

Job Summary: We are looking for a skilled Occupational Therapist to provide patient assessments, develop personalized therapy plans, and deliver evidence-based treatments in a home health setting.

Responsibilities
  • Conduct comprehensive patient assessments to identify functional status, occupational therapy needs, and adaptive equipment requirements.
  • Develop and implement individualized therapy plans in accordance with physician orders and patient goals.
  • Deliver evidence-based treatments, including fine motor coordination exercises, home assessments, and adaptive equipment training.
  • Collaborate with healthcare teams to ensure seamless care coordination and optimal patient outcomes.
Requirements
  • Bachelor's degree in Occupational Therapy or related field.
  • Current licensure as an Occupational Therapist in state of practice.
  • CPR certification and driver's license.

Estimated Salary Range: $65,000 - $90,000 per year, depending on experience and location.
