Job Title: Case Manager - Registered Nurse

LeaderStat is seeking a skilled Registered Nurse - RN to fill a Case Management position in a dynamic healthcare setting.

Job Summary:

We are looking for a dedicated and compassionate Registered Nurse - RN to join our team as a Case Manager. As a Case Manager, you will be responsible for coordinating patient care, developing and implementing care plans, and ensuring seamless transitions between healthcare providers.

Responsibilities:
  • Coordinate patient care and develop individualized care plans
  • Collaborate with healthcare providers to ensure effective communication and coordination
  • Monitor patient progress and make adjustments to care plans as needed
  • Provide education and support to patients and their families
  • Document patient information and maintain accurate records
Requirements:
  • 1 year of experience working as a full-time Registered Nurse
  • Clinical experience within the last 2 years
  • Current State Licensure
  • Current Relevant Certifications (BLS, ACLS, etc)
