Job Description
We're seeking a skilled Rural Care Specialist to join our team. As a Rural Care Specialist, you'll be responsible for facilitating comprehensive care management to promote patient-centered, evidence-based interventions. You'll conduct regular patient outreach, education, follow-up, and maintain a patient panel under the supervision of a provider.

Your primary goal will be to empower patients to achieve their best health through techniques such as health coaching, self-management support, and motivational interviewing. You'll build connections and relationships with patients and caregivers to create a personalized plan of care that meets both medical and non-medical needs unique to each patient.

In addition to providing direct care services, you'll coordinate and facilitate patient care transitions, ensuring seamless continuity between hospital, rehabilitation, and home settings. You'll also navigate and support patients to assist with SDoH needs, acting as a patient advocate to assess concerns and provide support to needed resources.
