Job Description: As an Hospice Care Coordinator, you will be responsible for triage of after-hours patient and family phone calls, determining the need for services including assessments, symptom management, and care plan updates. You will also initiate communication with the attending physician, other interdisciplinary staff members, and agencies for optimal care coordination.

This role requires empathy and strong clinical judgment, as you will work closely with patients and families during their most vulnerable time. Your primary focus will be on maintaining accurate clinical records, preparing timely care plans, and making pronouncements of death while providing support to families.

Salary: $85,000 - $110,000 per year
