Primary Care Physician Assistant

Job Description:
This part-time role involves working as an Advanced Practice Clinician within the Home-based Medical Care interdisciplinary team. You will collaborate with patients, PCPs, and specialists to develop and carry out patient goals of care in all phases of the patient journey.
You will manage chronic illnesses, provide patient education, and document timely to close HEDIS quality gap measures and recapture HCCs.
Key Responsibilities:
- Manage complex elderly populations with multimorbid conditions
- Document timely and focus on closing HEDIS quality gap measures and recapturing HCCs
- Engage new patients with comprehensive geriatric assessments
- Triage, diagnose, and treat patients on an urgent basis in home
- Order and interpret tests, prescribe medications, and start treatment in the home
- Collaborate with patient's PCP and specialists as needed
