Job Description
The Senior Care Manager provides high-quality care and services to residents by administering medication and treatments in a safe, organized manner. The role requires reviewing, reading, notating, and initializing the Daily Log to document pertinent resident information.
Responsibilities:
- Administer medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass.
- Ensure that medications are passed according to times, document, and initial as medications are given.
- Maintain confidentiality of all resident information, including medication details.
- Report resident concerns made while administering medication to the Resident Care Director or Wellness Nurse.
- Restock medication carts after each medication pass.
- Assist in checking medication regardless of packaging system.
- Assess residents to determine need for 'as needed' medication and document accordingly.
- Count all narcotics with another Medication Care Manager or Lead Care Manager each shift.
- Clean and maintain the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
- FOLLOW re-fill process for medications.
Requirements:
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential.
- Certifications, such as CPR and First Aid, must be maintained.
- Previous experience working with seniors is preferred.
- A desire to serve and care for seniors.
- Ability to make choices and decisions in the resident's best interest.
- Ability to remain calm in difficult situations.
- Handle multiple priorities effectively.
- Demonstrate good judgment, problem-solving, and decision-making skills.
