Local Contract Nurse RN Home Health

Healthcare Support is seeking a skilled and experienced local contract nurse RN Home Health to join our team.
Job Summary:We are looking for a dedicated and compassionate RN to provide high-quality care to patients in their homes. As a local contract nurse RN Home Health, you will work closely with our team to deliver exceptional patient outcomes and exceed our clients' expectations.
Responsibilities:- Provide comprehensive home health care services to patients in their homes
- Develop and implement individualized care plans to meet patients' unique needs
- Collaborate with healthcare teams to ensure seamless care coordination
- Conduct thorough assessments and evaluations to identify patients' needs and goals
- Document patient progress and outcomes in a timely and accurate manner
- Active RN license (Oregon)
- 1+ year of recent geriatric Home Health experience required
- Homecare Homebase + OASIS-E experience required
- BLS Certification from the American Heart Association
