Job Description

We are seeking an experienced Medical Care Navigator to join our team at the Illumination Foundation. This role will be responsible for ensuring the smooth discharge and oversight of medical care for homeless clients from hospitals or other agencies contracted with us.

Main Responsibilities:

  • Provide backup support to the medical team when needed, covering shifts due to staffing shortages or site needs.
  • E nsure completion of client documentation in KIPU and any external platforms in an accurate and timely manner by the medical team members.
  • Assist the medical team in maintaining client workflows to ensure clients' needs are met.
  • Attend meetings and relay pertinent information to medical team members.
  • Provide support to team members, listening to concerns/feedback and resolving conflict/issues, under the guidance of the Site Manager.

Client Care:

  • Conduct initial intakes to evaluate clients' fitness for recuperative care and identify outstanding medical needs.
    • Document medical and mental health status, check vitals, determine insurance status, and schedule follow-up appointments as outlined in discharge paperwork.
  • Collaborate with case management to link clients to medical services, including insurance, primary care providers, specialists, and referrals.
  • Coordinate home health visits if necessary.
  • Promote health literacy by educating patients on disease processes, medications, preventative measures, treatment management, and general health.
  • Arrange transportation for clients to and from medical appointments.
  • Report emergency situations to the Supervisor/Manager and document incidents, ensuring proper level of care and initiating medical response systems when necessary.

Administrative/Record Keeping:

  • Enter client information and case notes into our EHR and other documentation systems.
  • Perform administrative tasks such as answering phones and filing paperwork.
  • Maintain proper inventory of medical supplies and medications.

Compliance/Safety:

  • Maintain confidentiality according to HIPAA guidelines and DHS standards.
  • Provide clients with compliance forms upon admission to ensure program rules adherence.
  • Ensure care is provided using infection control measures protecting staff and clients, adhering to OSHA standards.
  • Conduct medication reconciliation and manage biomedical/pharmaceutical waste disposal.
  • Maintain a safe work environment by ensuring cleanliness of the med room, wound care room, and med storage room.
  • Ensure medical equipment remains operational.

Requirements:

  • High School Diploma or equivalent.
  • Current and non-restricted LVN license in California.
  • 3 years of relevant experience.
  • Basic computer skills, including email and spreadsheet summarization.
  • Valid CA Driver's License.

Salary and Benefits:

We offer a competitive salary range of $65,000-$80,000 per year, depending on experience. Additionally, we provide comprehensive benefits, including medical insurance, dental and vision coverage, life, AD&D, and LTD insurance, employee assistance programs, professional development reimbursement, 401K with company matching, 10 days vacation PTO, 6 days sick PTO, and 10 days holiday PTO. Potential eligibility for the Public Service Loan Forgiveness Program (PSFL) for federally qualified loans.
