AvaMed Workforce is seeking a skilled Physical Therapist to join our team at a correctional facility in Chowchilla, CA.
Key Details:
- Full-Time assignment
- 40 Hours per week
- Monday-Friday, 7:30am-4:00pm (1 week of orientation required)
- Rate: $55-$60/hr ($2,200-$2,400/week)
- Status: Independent Contractor
Responsibilities:
Under the direction of the Institution CEO/CME or designee, the Physical Therapist will perform care consistent with the scope of licensure and in accordance with the CDCR Institution/Facilities policies and procedures and consistent with generally accepted evidence-based Physical Therapy practices.
The Physical Therapist will provide Physical Therapy and evaluations on an "as needed" basis on CDCR patient/youth as pre-scheduled by designated Institution/Facility CEO/CME or designee.
The Physical Therapist will provide Physical Therapy services to patient/youth upon physician referral and order, or upon direct access to the Physical Therapy services in cases where the patient/youth is seeking Physical Therapy for an existing and documented diagnosis.
The Physical Therapist will develop a treatment plan consistent with those findings and be responsible for communicating the treatment plan to the patient/youth and any relevant medical personnel.
Requirements:
- Must have a Physical Therapist license by the California State Board of Physical Therapy
