Home Health Aide/Certified Nursing Assistant

Job Summary:
Northern Light Health is seeking a skilled Home Health Aide/Certified Nursing Assistant to provide high-quality care to our patients in their homes. As a Home Health Aide, you will be responsible for providing personal care and support to patients, including bathing, dressing, and grooming. You will also assist with meal preparation, light housekeeping, and other tasks as needed.
Responsibilities:
- Provide personal care and support to patients, including bathing, dressing, and grooming.
- Assist with meal preparation and light housekeeping tasks.
- Monitor patients' health status and report any changes to the primary clinician.
- Collaborate with the patient care team to develop and implement individualized care plans.
- Follow established procedures and standards to limit the spread of infection.
- Attend education sessions and workshops to enhance or gain new skills.
- Meet established productivity benchmarks.
- Work to ensure a safe environment for staff and patients.
Requirements:
- Certified as a Home Health Aide or Certified Nursing Assistant.
- Valid driver's license with reliable transportation and agency required auto liability insurance.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to lift, move, and load 30 to 50 pounds.

Potential exposure to noxious odors. Potential exposure to very hot or cold temperatures. Work with computers, typing, reading or writing. Work beyond the regularly scheduled hours. Extend body and limbs to reach items. Prolonged periods of kneeling. Alternate shift schedules (day, evening, nights, weekends). Prolonged periods of standing. Prolonged periods of walking.
