Director of Discharge Planning and Social Services

Regency Health & Rehabilitation Center in Yorktown, Virginia is seeking an experienced Director of Discharge Planning and Social Services to assist residents and families in preparing for their transition home. The ideal candidate will coordinate patient discharges, including supplies, transportation, and other services, to ensure a safe transition.

We are looking for a dynamic and self-motivated individual with 2 or more years of experience in Discharge Planning and Social Services in a healthcare setting. The ideal candidate has a history of successful positions involving direct customer service, social services, coordination, and problem-solving, and teamwork. Strong communication and collaboration skills are essential.

Key Responsibilities:

  • Research and arrange external services for residents, such as Chaplaincy, Advocates, and optimal care environments.
  • Schedule discharge meetings and coordinate home visits, home care, and therapy according to resident needs.
  • Direct timely and efficient five-day discharge planning meetings for all newly admitted residents.
  • Assess patient support systems and home situations to assist in decision-making regarding discharge plans and needs.
  • Facilitate effective bed management by strategic evaluation and internal arrangement to maximize availability of transitional and extended care services.
  • Meet with and assist residents with social issues, such as conflict resolution, problem-solving, and care coordination.
  • Conduct customer satisfaction surveys.

Requirements:

  • 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or other human services-related degree.
  • At least 1 year of Social Work experience in a healthcare setting.
