We are seeking an experienced Hospice Admissions Registered Nurse to join our team. This individual will be responsible for evaluating and admitting patients according to Local Coverage Determinations (LCD) Guidelines provided by the Medicare Administrative Contractor (MAC).
- Evaluating patients and developing an individualized plan of care that meets their unique needs.
- Cordinating patient care services as necessary, including arranging for medical equipment, supplies, and other support services.
- Documenting comprehensive assessments at point of care, ensuring accurate and complete records are maintained.
To be successful in this role, you must have:
- A current RN licensure within the state of branch location, with a minimum of one year of clinical nursing experience.
- The ability to work independently and possess strong communication, organization, and time management skills.
- A valid driver's license and a reliable vehicle for travel.
- Proficiency in using a personal computer and basic software applications, including Microsoft Office and electronic health record systems.
