Job SummaryWe are seeking an experienced MDS Coordinator to oversee and ensure the completion of the RAI/MDS process. This role is responsible for Medicare and MDS process management, including conducting patient assessments, developing patient care plans, making patient and facility recommendations, and overseeing Medicare and Medicaid programs.
Key Responsibilities- Conduct thorough patient assessments to inform care plans
- Develop and implement patient care plans that meet individual needs
- Make recommendations to improve patient and facility outcomes
- Oversee Medicare and Medicaid programs to ensure compliance and quality
- Provide support to facility staff to ensure seamless care delivery
- Associate's degree (Bachelor's preferred)
- Licensed as a Registered Nurse through the State of Minnesota
- MDS certification
- Medical assessment and patient care experience
- Attention to detail and critical thinking skills
- Organization and communication skills
