Job Summary:

The Physical Therapist will be responsible for assessing and evaluating patients' therapeutic, rehabilitative, and functional status. You will develop and implement individualized plans of care, providing physical therapy services to patients according to a written physician's plan. Your expertise will also include instructing patients and caregivers in the use and care of therapeutic appliances, determining priority needs for physical therapy, and reporting patient reactions to treatment or changes in condition.

Responsibilities:
  • Assess and evaluate patients' therapeutic, rehabilitative, and functional status
  • Develop and implement individualized plans of care
  • Provide physical therapy services to patients according to a written physician's plan
  • Instruct patients and caregivers in the use and care of therapeutic appliances
  • Determine priority needs for physical therapy
  • Report patient reactions to treatment or changes in condition
  • May train patients in the use of prosthetic devices
  • Identify patient and family/caregiver needs for other home health services/hospice and refer as necessary
  • Prepare and submit clinical and progress summaries based on the attainment of goals
  • Participate in discharge planning for patients
  • Provide physical therapy consultation to home families/caregivers when indicated
  • Provide in-service education programs for nursing organization personnel as needed
  • Participate in peer consultation process
Requirements:
  • BLS/BCLS certifications
  • Valid state license (where applicable)
  • SNF experience - highly preferred
