Responsibilities
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for clients.
- Provide occupational therapy services to a diverse range of clients, including those with physical, cognitive, or emotional disabilities.
- Develop and implement personalized care plans to help clients achieve their goals and maintain independence.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
Requirements
- Current Vermont OT licensure
- Experience in home health care, preferably with a focus on occupational therapy
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Preferred Qualifications
- McKesson/Netsmart experience
- Availability for flexible scheduling, including weekends and holidays
