Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Concord, NH. As a Physical Therapist, you will be responsible for providing high-quality care to adult and geriatric clients in their homes.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement evidence-based interventions to promote wound healing and prevent further injury.
  • Collaborate with nursing staff to develop and implement comprehensive care plans.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
  • Document client progress and care management using electronic medical records.
Qualifications:
  • A current New Hampshire Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and experience with electronic medical records.
