The Care Partner serves as a vital support role under the supervision of the Registered Nurse. The primary objective is to enhance the patient experience by collaborating with the nursing staff, physicians, and the healthcare team to deliver outstanding patient care. Responsibilities include assisting patients with daily living activities and comfort measures, while ensuring safe and culturally competent care.
The role may involve advanced technical training skills such as phlebotomy, simple treatments, dressing changes, oral suctioning, and the discontinuation of Foley catheters, depending on unit needs. Clerical tasks such as data retrieval, maintaining unit supplies, and transporting medications will also be part of the role. The Care Partner must thrive in a fast-paced environment, demonstrating effective communication and multitasking abilities while working under pressure with a compassionate and respectful approach.
Core Responsibilities and Essential Functions
Excellence in Patient Experience: Collaborate with the healthcare team, patients, and families to ensure a patient-centered care environment. Responsibilities include:
- Protecting patient rights and dignity.
- Maintaining confidentiality in accordance with HIPAA regulations.
- Participating in hourly patient rounding.
- Responding promptly to telephone and call lights, notifying the appropriate personnel as needed.
- Addressing patient concerns or escalating them to the charge nurse when necessary.
- Offering assistance and comfort measures proactively.
- Presenting oneself in a professional manner, showing sensitivity to the needs of others.
- Providing patient care under the guidance of the RN.
- Assisting with hygiene, nutrition, elimination, and physical comfort measures.
- Supporting the RN and healthcare team with care delivery as assigned.
- Monitoring data entry for key patient safety information.
- Participating in charge reconciliation and performing audits as requested.
Support the initiatives of the healthcare team and engage in unit performance improvement efforts. Responsibilities include:
- Participating in shared leadership and ethical accountability.
- Transporting supplies and equipment as assigned.
- Practicing accountability in all tasks.
Utilize strong verbal and written communication skills with all stakeholders, including patients, staff, and families. Responsibilities include:
- Documenting patient care in the Electronic Medical Record (EMR).
- Communicating patient care concerns to the RN or Charge Nurse in a timely manner.
- Maintaining and managing patient care equipment.
Ensure a safe environment for patient care by:
- Consistently practicing proper hand hygiene.
- Keeping patient areas clean and organized.
- Following infection control policies and utilizing proper body mechanics.
High school diploma or GED is required. Graduation from a State-approved program for Certified Nursing Assistants or Patient Care Tech Program is preferred.
Required Minimum License(s) and Certification(s)
All certifications must be obtained upon hire unless otherwise stated:
- Basic Life Support (BLS) certification.
- Certified Nurse Assistant certification is preferred.
Ability to follow detailed instructions, strong interpersonal skills, and effective communication are essential. Candidates must be able to work independently and as part of a team. The minimum age for hire is 16.
