Responsibilities- Provide skilled nursing services to patients in their homes, including wound care, IV therapy, and medication management.
- Conduct initial evaluations and regularly reevaluate patient needs to ensure the highest level of care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Document patient progress and communicate effectively with patients, families, and healthcare teams.
- Graduate of an accredited school of nursing and currently licensed in the state of Texas.
- Minimum 1 year of experience in home health nursing, with a focus on wound care and IV therapy.
- Strong clinical skills and ability to work independently with minimal supervision.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
