Kaiser Permanente seeks a seasoned professional to lead our Social Services Case Management team. As a key member of our healthcare organization, you will be responsible for developing and implementing social services programs that meet the needs of our patients and members.
Key Responsibilities:- Program Development: Plan, develop, and implement social services policies and procedures, delivery of social work programs, and discharge planning services.
- Team Leadership: Oversee social services case management activities, manage staff activities, and ensure their competencies.
- Program Evaluation: Develop and manage departmental budget, identify opportunities for cost reduction and improved services, and monitor the quality assurance of social services programs and staff.
- Community Collaboration: Identify and implement opportunities for collaboration with community organizations to develop new programs or enhance current community services available to Kaiser members.
- Regulatory Compliance: Ensure compliance with federal, state, and local requirements.
- Leadership Experience: Minimum two (2) years of leadership or supervisory experience in medical social work.
- Education: Masters degree in Social Work from an institution accredited by the Council of Social Work Education.
- Licensure: Licensed Clinical Social Worker (California) required at hire.
- Advanced Leadership Experience: Minimum five (5) years of supervisory experience in medical social work preferred.
