Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Interim HealthCare of the Twin Cities. As a Home Care LPN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Participate in ongoing interdisciplinary assessment of the patient
  • Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s)
  • Provide services that are ordered by the physician as indicated in the plan of care
  • Recognize and report changes in the patient's condition to the RN supervisor
  • Counsel and educate the patient and family in meeting healthcare-related needs
  • Complete timely and accurate clinical notes, including addressing patient progress
Requirements
  • Current license as a Licensed Practical Nurse (LPN) in the State of MN
  • License is active and in good standing
  • 2 years' experience as an LPN
  • Able to travel within geographic area serviced by the office
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • CPR Certification
