Job Summary:

- Provides professional nursing services or supervises the provision of nursing services to hospice patients in accordance with accepted standards of practice and organizational guidelines.">
- May perform nursing services in a variety of care settings and/or roles, including case management, admissions, or on-call.">
Key Responsibilities:
">- ">
- Conducts thorough physical assessments and medical histories to determine patient needs and develop individualized care plans.">
- Works collaboratively with the interdisciplinary team to prioritize and provide patient-centered hospice services, addressing complex emotional, spiritual, and environmental needs.">
- Communicates effectively with physicians, social workers, and other healthcare professionals to ensure seamless care delivery and address any concerns or issues that arise.">
- Provides end-of-life care, comfort care, and symptom management according to established indicators and protocols.">
- Educates patients, families, and caregivers on disease processes, self-care techniques, and end-of-life care, promoting informed decision-making and empowerment.">
- Coordinates ordering durable medical equipment and supplies, ensuring timely access to necessary resources.">
- Participates in quality assurance processes, internal/external education, marketing, and other initiatives that promote the organization's mission.">
Requirements:
">- ">
- Graduation from an accredited school of nursing with a current license to practice as a registered professional nurse in Michigan.">
- Minimum one to two years of varied nursing experience; hospice experience and CHPN certification preferred.">
- Ability to use technology required for clinicians in their practice.">
- Excellent interpersonal skills, sound judgment, effective organizational, prioritization, and follow-through skills, attention to detail, tact, dependability, emotional intelligence, and ability to maintain confidentiality and promote positive relationships.">
- Must be able to cope with family/caregiver emotional stress, cultural diversity, and individual lifestyles.">
- The physical demands of the position include vision, effective speech and hearing for extensive telephone contact; repetitive motion; traveling; driving or riding in a motor vehicle for extended periods; standing, sitting, walking, bending, reaching, and stretching; lifting up to fifty pounds unassisted and the ability to assist in lifting patients using appropriate lifting techniques and/or devices.">
- Proof of current tuberculin testing required. Patient contact will not be allowed until tuberculin clearance is documented.">
- Must be able to work variable hours/shifts and/or days, including weekends. Must have the ability to occasionally work extended days, as necessary.">
- Must have reliable transportation.">
- Must be eligible to work in the United States.">
Salary Range: $65,000 - $85,000 per year.
Estimated Annual Salary:**
">$70,000 (estimated).
