Job Overview:

About the Job:
This is an exciting opportunity to join our team and make a real difference in people's lives. As a Resiliency Support Specialist, you will be responsible for assessing client needs, developing service plans, and coordinating community resources to support clients in achieving their goals. You will also work collaboratively with referral sources, healthcare professionals, and other stakeholders to ensure seamless care coordination.

Key Responsibilities:
* Assess client needs and develop individualized service plans
* Coordinate community resources and services to support clients' goals
* Collaborate with referral sources and healthcare professionals to ensure seamless care coordination
* Monitor and evaluate client progress, updating service plans as needed

Requirements:
* BA in Psychology, Social Work, or a related Human Services field
* 3+ years of experience in mental health or a related field
* Valid driver's license and reliable vehicle required
* Ability to work in Hamilton and Clermont Counties
