We are seeking a highly skilled Physical Therapist to join our team at JAG Physical Therapy. As a Physical Therapist, you will play a critical role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
  • Develop and implement treatment plans based on patient evaluations and goals.
  • Collaborate with interdisciplinary teams, patients, and support systems to ensure effective care.
  • Provide education and training to patients and support systems on therapeutic activities, rehabilitation equipment, and community reintegration.
  • Oversee support staff, including PTA, ATC, and PT aides.
Requirements
  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements.
  • Valid CPR license.
  • Experience with EMR software preferred.
  • Excellent communication, clinical decision-making, and teamwork skills.
