We are seeking a skilled Speech Therapist to join our team at Your Tailor Made Senior Services. As a Speech Therapist, you will work with patients in their homes to help them achieve their functional goals and improve their quality of life.
Key Responsibilities- Design and implement individualized plans of care for patients based on thorough assessments of their needs and abilities.
- Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists as needed.
- Conduct therapy sessions within defined standards of care and direct the assistance of support staff to achieve patient goals.
- Communicate effectively with other professionals and support staff to ensure positive patient outcomes.
- Document evaluations, treatment goals, and plans via our EMR system, Finale Health, and maintain accurate and up-to-date patient records.
- Participate in ongoing training and education to maintain current knowledge and skills in therapy practice.
- Collaborate with our management team to maintain quality patient care and achieve agency goals.
- Master's degree in Speech-Language Pathology or related field.
- Current licensure and certification in Speech-Language Pathology.
- Experience working with patients in home health settings.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
