Key Responsibilities- Deliver direct and indirect patient care in accordance with hospital policies, procedures, and professional standards of care.
- Assist the Registered Nurse (RN) in collecting patient health data, implementing individualized care plans, and evaluating their effectiveness.
- Work collaboratively with the RN to reassess patient conditions, take appropriate action, and involve patients and families in care decisions.
- Document patient care activities accurately and timely, utilizing the electronic medical record.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and patient safety.
- Participate in performance improvement activities, data collection, and quality initiatives to enhance patient care and customer service.
- Graduate of an accredited School of Practical Nursing.
- Current licensure to practice as a Licensed Practical Nurse in the State of Pennsylvania.
- Basic Life Support (BLS) certification.
- Act 33, Act 34, and Act 73 clearances.
