Job Summary

ClearSky Health is seeking a skilled Licensed Vocational Nurse, LVN to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our staff to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Provide skilled nursing care to patients under the direction of a Registered Nurse and in accordance with physician's orders and established standards and protocols.
  • Implement orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the Registered Nurse.
  • Provide information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
  • Participate in evaluation of the effectiveness of interventions in relation to patient responses and seek guidance in redirection when necessary.
  • Report to supervisor on patient care rendered, problems concerning patients' needs, and resolution to those needs.
  • Help identify patient problems and potential risks and contribute to the development of interventions.
  • Assist in identification of patient's learning needs and reinforce self-care and other techniques.
  • Appropriately designate applicable charges to individual patients using the hospital's established system.
  • May be required to work during inclement weather and other staffing emergencies.
  • Provide an environment conducive to safety for patients, visitors, and staff. Assess the risks for safety and implement appropriate precautions. Comply with appropriate and approved safety and Infection Prevention standards.
  • Maintain regular attendance in keeping with assigned work schedule.
  • Perform other duties as assigned to support overall effectiveness of the organization.
Requirements
  • Two years' experience in inpatient hospital setting, inpatient rehabilitation experience preferred.
  • Current state license as a Licensed Vocational Nurse required.
  • Current CPR certification required.
Preferred Qualifications
  • Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
  • Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
  • Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients.
  • Demonstrates general computer skills including data entry, word processing, email, and records management.
  • Demonstrates critical thinking skills.
  • Effective organizational and time management skills.
  • Effective written and verbal communication skills.
  • Ability to prioritize, meet deadlines, and complete complex tasks.
  • Ability to maintain quality, safety, and/or infection prevention standards.
  • Ability to work independently.
  • Ability to maintain proper levels of confidentiality.
  • Ability to work closely and professionally with others at all levels of the organization.
Physical Requirements
  • A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.
  • Both gross and precise motor functions.
  • Lifting/exerting of up to 50 lbs.
  • Possible exposure to bodily fluids.
  • Visual acuity required for patient assessment and documentation of care.
  • Acute hearing required for accurate patient assessment.
  • Sufficient manual dexterity to operate equipment and computer keyboard.
  • Close vision and the ability to adjust focus.
