We are seeking a highly skilled and experienced Primary Care Provider to join our growing team at Fast Pace Health. As a Primary Care Nurse Practitioner or Physician Assistant, you will play a vital role in delivering high-quality patient care and contributing to our mission of providing exceptional healthcare services in rural communities.
Key Responsibilities- Treat patients and provide quality healthcare through direct contact and mediated communication.
- Interact closely with clinic members, including telehealth team members, other Primary Care Providers, and telehealth call center team members, to achieve excellent patient care.
- Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up.
- Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
- Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
- Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.
- Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
- Prescribe medications, recommend treatments, and provide written home-going instructions.
- Refer patients to specialists or other healthcare resources as needed.
- Educate patients and their families on health maintenance and preventive care.
- Promote patient engagement, health literacy, and shared decision-making.
- Participate in quality improvement initiatives and value-based care programs.
- Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships.
- Nurse Practitioner (NP) / Physician Assistant (PA) with 2+ years of dedicated experience in a direct primary care setting.
- Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures.
- Strong understanding of Patient-Centered Medical Home (PCMH) principles and value-based care.
- Willing to perform pediatric exams and see children ages 3 years and up.
- Masters Degree.
- Family Nurse Practitioner License, Physicians Assistant License, APRN.
