Crisis Stabilization Social Worker Role

This position at Dakota County involves providing short-term, intensive, in-home therapeutic services to individuals experiencing ongoing mental health crises. The Crisis Stabilization Social Worker is responsible for developing and implementing individualized treatment plans, providing direct services and advocacy to clients and families, and facilitating community resources on behalf of the client.

Key Responsibilities:
1. Develop and implement individualized treatment plans, defining initial and ongoing measurable goals and objectives.
2. Provide direct services and advocacy to clients and families, including crisis management and cultural competency.
3. Facilitate and mobilize community resources on behalf of the client, developing services where needed resources do not exist.
4. Evaluate, document, and monitor progress toward goals and objectives, including research and application of relevant policies, procedures, and statutes.
5. Complete agency, state, and federal required documentation as related to client activity according to mandated timelines.

Work Environment:
This role involves working in a County office building and on client home visits, with some travel required. The ability to lift up to 20 pounds on an occasional basis is necessary, as well as exposure to inadequate lighting on client home visits.

Selection Process:
The examination/selection process for this classification will consist of a rating of your training and experience from the application materials submitted. The top scoring candidates will be forwarded to the hiring department for further consideration.
