Key Responsibilities
The Home Health Patient Care Management Assistant (Licensed Practical Nurse, LPN) plays a crucial role in assisting the Patient Care Manager in coordinating a team to ensure the delivery of high-quality care to home health patients within the designated area.
- Ensures the team is dedicated to providing quality care efficiently, adhering to physician's orders and State/Medicare regulations.
- Prepares patient lists and necessary documentation for weekly case discussions.
- Delivers direct patient care as needed, following the scope of practice, care plans, and physician directives, in collaboration with other healthcare team members.
- Assists the Patient Care Manager in reviewing clinician documentation to ensure complete, accurate, and appropriate submission of all records by field staff.
License Requirements:
- Current CPR certification is mandatory.
- Active LPN licensure in the state of practice.
- Valid Driver's License, vehicle insurance, and access to reliable transportation or public transit.
