Job Summary

The RN - PACE LTSS Screening RN is a key member of the In-Home Services team, partnering with the local Enrollment team to determine participant eligibility for the PACE program. This role involves scheduling and conducting home visits, assessing participant needs, and collaborating with the InnovAge Primary Care Physician to ensure accurate documentation and enrollment.

Responsibilities
  • Level of Care Assessments: Schedule and conduct home visits to assess participant needs and determine eligibility for the PACE program.
  • Documentation and Collaboration: Complete documentation in close collaboration with the InnovAge Primary Care Physician and compile supporting documentation for enrollment.
  • Outreach and Engagement: Participate in outreach programs and assist with case management of peer review of client records.
  • Interdisciplinary Team Collaboration: Serve as a member of the Interdisciplinary Team (IDT) and coordinate appointments for prospects requiring further assessments.
  • Knowledge and Expertise: Maintain knowledge and expertise in eligibility, enrollment, and program specifications for Medicare and Medicaid, as well as the Program of All-Inclusive Care for the Elderly (PACE).
Requirements
  • Education: Associate degree from an accredited nursing school.
  • Licensure: Current Registered Nurses License in the state where working.
  • Certification: Current CPR certification or the ability to obtain CPR certification.
  • Experience: 3+ years of healthcare experience with an emphasis in geriatrics, and 1+ year of experience working with the frail or elderly.
  • Transportation: Requires transportation, a state-issued driver's license, good driving record, and auto insurance as required by law.
Preferred Qualifications
  • Education: Bachelor's degree in nursing.
  • Certification: Certification as a Gerontological Nurse.
  • Experience: Prior experience working with State and Federal Regulators, and bi-lingual skills.
