As a Home Health Registered Nurse with Mercy Medical Center, you will have the opportunity to provide high-quality, patient-centered care to adult and geriatric patients in their homes. Our team is dedicated to delivering exceptional care and making a positive impact in our community.
Responsibilities
• Conduct initial evaluations and regular re-evaluations to assess patient needs and develop individualized care plans
• Provide specialized care, including preventive, rehabilitative, and maintenance therapies, to patients with a range of conditions, including chronic and acute illnesses
• Ensure compliance with established care plans, accepted nursing standards, and physicians' orders
• Collaborate with assigned LVNs and Certified Home Health Aides to provide comprehensive care
• Identify and prioritize patients' biophysical, environmental, self-care, educational, and discharge planning needs
Requirements
• One year of Home Health nursing experience within the past 5 years, or equivalent experience in acute care Medical/Surgical Telemetry and/or ICU nursing
• Current Registered Nurse license with the state of California
• Basic Life Support (BLS) Certificate
• Valid California Driver's license and proof of auto insurance
Preferred Qualifications
• Public Health Home Health Nursing or Hospice experience
• Knowledge of Medicare, Medical, and private insurance, as well as local community resources
• BSN degree
• Second language skills in Spanish
