Job Summary

Freedom Home Health And Hospice Care Services Inc is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist, you will be responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Key Responsibilities

  • Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  • Assess home environments for hazards or barriers to independent living and identify equipment needs.
  • Develop treatment programs and establish goals for improved function, communicating plans of care to referring physicians and other healthcare professionals.
  • Teach new skills or retrain patients in daily activities lost due to illness or injury, in accordance with Organization policy.
  • Fabricate splints and instruct patients in the use of adaptive equipment to improve function.
  • Train patients in the use of prosthetic and/or orthotic devices.
  • Maintain accurate clinical records, clinical notes, and reports to physicians on patient condition changes.
  • Collaborate with other healthcare personnel to promote coordinated, efficient care.
  • Participate in inservices, case conferences, and meetings to ensure comprehensive plans of care.
  • Identify patient and family/caregiver needs for other home health services and refer as appropriate.
  • Supervise certified occupational therapy assistants according to organization policy and state regulations.
  • Participate in discharge planning for patients.

This position requires a graduate degree in occupational therapy from an accredited program, registration with the National Registration Examination of the American Occupational Therapy Association, and two years of clinical experience as an occupational therapist. The ideal candidate will possess excellent communication and organizational skills, as well as current CPR certification.
