Healthcare Navigator

We are looking for a Healthcare Navigator to join our team at UnitedHealth Group. In this role, you will provide health education, coaching, and treatment decision support for our members, while also identifying, coordinating, and/or providing appropriate levels of care under the direct supervision of an RN or MD.
The ideal candidate will have a high school diploma or GED, an unrestricted LPN or LVN license in the state where the position will be performed, and 3+ years of experience in case management or acute care. They should also possess basic computer proficiency and be able to travel 30-50% of the time to local hospitals on a daily basis.
The estimated salary for this role is $26.54 per hour, with the possibility of reaching up to $34.64 per hour based on various factors such as education, work experience, certifications, etc.
