Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Longevity Health. As an Advanced Practice Provider, you will play a critical role in delivering high-quality, patient-centered care to our members in long-term care facilities.

Key Responsibilities
  • Provide comprehensive primary care services to a panel of Longevity Health members, including geriatric and palliative care.
  • Develop and implement individualized care plans, working closely with the care team to ensure seamless coordination of care.
  • Conduct regular assessments, diagnose, and treat members' medical conditions, prescribing medications as needed.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure effective communication and coordination of care.
  • Participate in care team meetings, providing updates on members' health status and care plans.
  • Communicate regularly with members, their families, and caregivers to ensure their needs are met and their goals of care are understood.
  • Stay up-to-date with the latest clinical guidelines, best practices, and regulatory requirements to ensure high-quality care.
Requirements
  • Master's degree in Nursing from an accredited program.
  • Current licensure as a Nurse Practitioner in the state of practice.
  • Board certification in Adult, Family, Geriatric, or Primary Care nursing.
  • Minimum of 6 months of experience as a Nurse Practitioner, with some experience in geriatric or palliative care.
  • Excellent communication, interpersonal, and clinical skills.
  • Ability to work autonomously and as part of a team.
