Position Overview:
Under the supervision of a Registered Nurse and in accordance with medical directives from healthcare practitioners, the Licensed Practical Nurse (LPN) delivers both direct and indirect nursing services within an outpatient environment.
Key Responsibilities:
1. Prepares and reviews medical documentation for assigned patients; initiates follow-up on medical records as required; assesses patient needs; identifies and organizes necessary orders for patient care, obtaining authorization from the practitioner as needed; accepts verbal orders in compliance with established policies; monitors, gathers, and records new patient information; communicates with the RN or practitioners regarding any missing data, new observations, or abnormal findings.
2. Executes fundamental nursing procedures as outlined in the care plan; administers treatments, immunizations, and medications as prescribed; prepares medications for practitioner use; conducts point-of-care testing as permitted; provides laboratory, radiology, and other reports to practitioners for evaluation; informs patients of normal test results as directed; considers the unique needs of various age groups when delivering care; prioritizes care based on patient acuity.
3. Accurately documents the implementation of orders and interventions; records findings, instructions, and patient responses using appropriate forms or electronic records in accordance with departmental and JCAHO standards.
4. Monitors patient and family responses to the care plan; reports unexpected reactions to the RN or practitioners; collaborates with the RN or practitioner to adjust the care plan as necessary.
5. Demonstrates and applies proficiency in essential nursing skills and equipment usage; maintains up-to-date knowledge and skills required for the role; participates in staff meetings and engages in projects as assigned.
6. Responds effectively in emergency situations within the clinic; adheres to established protocols; informs the appropriate practitioner or RN.
7. Delegates basic nursing tasks to unlicensed personnel as appropriate, ensuring clear instructions are provided; acts as a resource for unlicensed staff; follows up on assigned care to ensure compliance with standards, policies, and procedures.
8. Implements discharge plans developed by the practitioner or RN; processes and follows up on referrals as directed; provides educational materials and pre-printed information to patients as instructed by the RN or practitioners, documenting patient and family responses to the instructions and addressing any concerns.
9. Manages patient immunization data, including updates to the Hawaii Immunization Registry System (HIR) and Electronic Health Records (EHR), ensuring all records are accurate at each patient visit. Prepares immunization information prior to patient visits and administers vaccines according to health plan and VFC guidelines.
10. Identifies immunization needs based on age-specific guidelines from the CDC; completes necessary immunization documentation for record-keeping; oversees vaccine inventory control at the assigned clinic, including ordering and maintaining appropriate stock levels.
11. Ensures compliance with infection control policies and medication administration protocols, adhering to health plan, CDC, state, and federal VFC program guidelines.
12. Provides educational resources to patients and reviews instructions collaboratively with the RN or practitioner, adjusting the teaching plan as necessary.
13. Participates in the orientation of new staff members, demonstrating the necessary knowledge, skills, and abilities in the area of specialization. Maintains clean and sterile techniques, prepares for procedures, and assists with clinical tasks within the scope of practice.
14. Responsible for the organization, rotation, and stocking of supplies; schedules patients for routine and acute care while addressing care gaps; prepares and processes patient documentation.
Basic Qualifications:
Experience: N/A
Education: Completion of a Hawaii State Board of Nursing approved curriculum for LPN licensure.
License, Certification, Registration: Current Licensed Practical Nurse (Hawaii) license required upon hire; Basic Life Support certification from the American Heart Association.
Additional Requirements:
Demonstrated skills in conflict resolution, customer service, interpersonal communication, problem-solving, teamwork, and written communication.
Preferred Qualifications:
Prior nursing experience; previous experience in a clinic setting; proficiency in word processing and spreadsheet applications.
