About the Role: As an LPN at Washington Square, you'll be responsible for delivering high-quality patient care, working collaboratively with healthcare professionals to ensure optimal outcomes. Your skills and expertise will make a significant difference in the lives of our residents.

Responsibilities Include:

  • Validating patient information and administering medications as prescribed
  • Conducting vital sign checks and reporting any changes or concerns
  • Providing wound care and other treatments as needed
  • Collaborating with healthcare teams to develop and implement care plans
  • Maintaining accurate and detailed records of patient interactions and treatment progress

Requirements:

  • A valid Ohio nursing license or multi-state license including Ohio, in good standing
  • Reliable transportation to ensure timely attendance and response to emergencies
  • Successful completion of facility-conducted orientation, mandatory training, and in-service programs
  • Prior long-term care experience preferred, but not required
  • Strong organizational and communication skills to effectively collaborate with colleagues and residents
