About the RoleThe Community Case Manager is responsible for ensuring that program participants receive meaningful, goal-directed services tailored to their individual needs, interests, and desires. We offer Targeted Community Case Management Services focused on meeting the diverse needs of adults with intellectual disabilities and autism.
Key Responsibilities:- Routinely communicate with service recipients, their families, and guardians.
- Develops, writes, and updates service plans, reports, and correspondence.
- Organizes and facilitates Person-Centered Planning, service review meetings that include the individual, service providers, and other supports.
- Collects and compiles relevant case history information.
- Conducts intake and orientation for new participants to Community Case Management services.
- Supports and monitors individual access and maintenance of benefits and entitlements, including social security, MaineCare, waiver funding, mental health services, and resources needed.
We seek candidates with a Bachelor's Degree from an accredited institution in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing, or a closely related field. Alternatively, a Bachelor's Degree in an unrelated field with at least one year of full-time equivalent relevant human services experience is also acceptable. LSW or LSX certification is preferred. A valid driver's license, satisfactory driving record, and ability to speak, read, and write in English are essential requirements.
