Key Responsibilities- Work collaboratively with a DispatchHealth Medical Technician (DHMT) to deliver comprehensive care to patients in their homes.
- Perform thorough health assessments, diagnose, and treat complex illnesses, and provide therapeutic interventions as needed.
- Utilize critical thinking skills and evidence-based standards of practice to ensure the highest level of care.
- Accurately document patient encounters and maintain accurate records.
- Analyze test data to diagnose, treat, and arrange follow-up care for patients.
- Educate patients and families on managing their health and well-being.
- Navigate complex conversations related to end-of-life issues and goals of care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Identify and proactively solve problems to ensure the best possible outcomes.
- Adhere to clinical and safety standards, protocols, and performance metrics.
- Provide compassionate, culturally sensitive care that meets the unique needs of each patient.
- Minimum of two years of experience as an Advanced Practice Provider in acute care settings, such as Emergency Departments, Urgent Care, or Internal Medicine.
- Graduate of an accredited institution with an advanced degree.
- Current, unrestricted state license as a Nurse Practitioner or Physician Assistant.
- Nationally board certified, current.
- Current BLS certification, with ACLS certification preferred.
- Prescriptive authority and DEA registration.
- Valid driver's license with a clean driving record.
- Ability to lift and carry equipment up to 50 pounds.
- Ability to walk up and down stairs easily while carrying equipment.
- Ability to work a varied schedule, including evenings, holidays, and weekends.
