Job Summary

We are seeking a highly skilled and compassionate Patient Care Technician to join our team at Northwestern Medicine. As a key member of our interdisciplinary rehabilitation care team, you will play a vital role in supporting the care and recovery of our patients.

Key Responsibilities
  • Collect and report pertinent physiological, psychological, and functional data to inform patient care and treatment plans.
  • Assist patients with activities of daily living, including personal hygiene, mobility, and skin care, promoting maximal independence and skill practice.
  • Perform preventive skin care programs and report any changes in skin integrity to the registered nurse.
  • Assist patients with bladder and bowel training programs, including post-void residuals and spot bladder scans, and perform straight catheterization for select patient populations under the direction of the registered nurse.
  • Obtain and document patients' vital signs and pulse oximetry, and communicate any reportable values, symptoms, or signs of distress to the registered nurse.
  • Assist patients with transfers and mobility activities, utilizing appropriate techniques, precautions, and assistive devices, and practice therapy transfers and ambulation with patients within safe patient handling guidelines as delegated by the registered nurse.
  • Assist patients with the application of assistive devices and prosthetics/orthoses according to individual patient plans of care.
  • Assist patients with completing menus, passing patient meal trays and HS snacks, verifying any modified diet as ordered, and accurately calculating and documenting calorie count and carbohydrates, intake, and output as ordered.
  • Assist patients with 1:1 feeds as ordered, and cue patients to use ordered swallowing strategies under the direction of the registered nurse.
  • Answer and triage patient call lights and meet patient care needs with an appropriate sense of urgency.
  • Proactively set up patient rooms for therapy readiness.
  • Get weekly weights on all patients, and daily as ordered, using appropriate methods for patients' mobility, and escalating changes as needed.
  • Collect laboratory specimens as directed by the registered nurse.
  • Obtain blood glucose measurements and 12-lead EKGs as ordered.
  • Transfer patients from wall to tank oxygen or tube feedings in preparation for patient attendance at therapy.
  • Provide supervision and activities, or restraint use, for patients with high risk for fall or injury.
  • Regular comfort and environmental safety rounding to proactively meet patient needs.
  • Provide de-escalation techniques for agitated and aggressive patient behaviors.
  • Serve as a safety 1:1 for patients as needed.
  • Maintain and organize unit equipment inventories.
  • Precept new employees and partner with RNT-certified nursing students during clinical rotations.
  • Inclusive of all other duties as assigned by the nurse manager, nurse supervisor, staff nurse, and charge nurse.
  • Serve on the Cardiac Arrest team performing high-quality CPR.
  • Participate in bedside handoff.
Quality Improvement
  • Participate in system and local quality improvement goals.
  • Collect data and audits for Joint Commission readiness and improvement projects.
  • Assist in the maintenance of unit readiness to meet regulatory standards.
Communication
  • Maintain a therapeutic relationship to provide optimal customer-focused care.
  • Communicate effectively with patients at a level appropriate to age and cognitive status, showing cultural sensitivity.
  • Interact effectively with peers and other hospital staff to foster positive working relationships.
  • Effectively triage patient and general concerns to the registered nurse.
Infection Control/Patient Safety
  • Contribute to the safe, effective, and professional delivery of patient care, including the use of safety/restrictive devices.
  • Follow Infection Control policies and procedures.
  • Maintain knowledge of Northwestern Medicine's policies concerning precautions, codes, conditions, warnings, and general safety procedures.
Adaptability and Flexibility
  • Rotate to all units based on patient care needs.
  • Adjust to changes in policy and procedure, unit, and patient population.
Qualifications
  • High School or equivalent (GED) High School degree, or equivalent, is required.
  • Current Nursing Assistant certification (is required or current enrollment in an RN Nursing Program, with the successful completion of two clinical rotations).
  • Maintain current CPR certification.
  • Basic math skills are required.
  • Ability to communicate with a wide variety of people and provide the highest level of customer service.
  • Understanding and support of Northwestern Medicine's Mission and Values.
  • Flexibility and ability to manage work in a changing environment.
  • 12-lead EKG competency completed within 6 months of employment.
Preferred Qualifications
  • 2-5 years of work experience is preferred in a rehabilitation hospital, acute care hospital, or skilled nursing facility.
