Compensation Package

About the Role

This position requires a skilled Licensed Practical Nurse (LPN) to provide advanced clinical services and emotional support to patients recovering from illness, injury, or surgery in the comfort of their own homes. As a member of our team, you will be responsible for implementing plans of care, evaluating treatment outcomes, and documenting patient interactions electronically within 24-48 hours.

Key Responsibilities
  • Conduct regular home visits to assess patient needs and develop personalized care plans
  • Implement and evaluate treatment plans, ensuring seamless coordination with other healthcare professionals
  • Provide compassionate and empathetic support to patients and their families
Requirements

To succeed in this role, you will require:

  • A valid LPN license in New York State
  • At least one year of Med/Surg experience in a hospital or long-term care facility
  • A valid driver's license and reliable vehicle
  • Bilingual skills in Spanish, Mandarin, or Cantonese are highly desirable
