As a key member of our multidisciplinary team, you will be responsible for developing, teaching, and evaluating effective delivery of home patient care and compliance with clinical policies and procedures.

Key Responsibilities:

  • Education and Training: Educate and train patients on PD (CAPD / CCPD) and home dialysis, ensuring they understand the modality options and home dialysis requirements.
  • Coordination and Collaboration: Coordinate with multidisciplinary team members to ensure seamless patient care at home, including supply companies, primary care physicians, and transplant teams.
  • Patient Evaluation and Liaison: Meet and evaluate new home patients, explaining modality options and home dialysis requirements, and serving as a liaison between patients, supply companies, primary care physicians, and transplant teams to ensure continuity of care.
  • Equipment and Water Quality: Become proficient in understanding and documenting dialysis equipment and water quality in the home, ensuring patient safety and compliance with clinical policies.
  • Care Plan Meetings and Charting: Participate in care plan meetings, charting to maintain complete and accurate records, and communicate patient needs to direct supervisors and physicians.
  • Patient Interaction and Communication: Interact with patients and their families to provide instruction and assess daily patient care needs, and communicate patient needs to direct supervisors and physicians.
  • Office Visits and Order Implementation: Participate in patient office visits with physicians, accurately receiving and implementing new physician orders, and relaying order changes to patients and caregivers.
  • Test Scheduling and Results Communication: Schedule, obtain, review, and communicate test results to physicians, and relay order changes to patients and caregivers.
  • Record Keeping and Reporting: Complete required clinic records, including occurrence reports, CQI projects, primary nurse duties, and daily computer entry and reporting requirements.
  • Education and Professional Development: Complete and attend required educational programs, ensuring ongoing professional development and expertise in home dialysis and patient care.

Qualifications and Requirements:

  • Education and Licensure: Graduated from an accredited Registered Nurse program with current Alaska license or ability to obtain.
  • Experience: 1 year of nursing experience required, with dialysis experience preferred.
  • Certifications: BCLS and ACLS certification preferred.
  • Leadership and Organizational Skills: Demonstrated leadership ability and organizational skills, with the ability to communicate effectively and understand and implement government standards.
  • Computer Software Knowledge: Proficient in computer software and technology, with the ability to learn and adapt to new systems and processes.

Physical Job Requirements:

  • Physical Demands: Frequent long periods of standing, bending, stooping, walking, and sitting, with the ability to lift up to 30 lbs.
  • Travel Requirements: Valid driver's license, with occasional travel by airplane required, and flexibility to travel between clinics and patients' homes.
