The Emergency Department Care Manager at Christus Health is a critical role that requires a unique blend of clinical expertise, business acumen, and interpersonal skills. As a key member of the interdisciplinary team, the Care Manager will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the most appropriate level of care.
Key Responsibilities- Conduct thorough patient assessments to determine the most suitable level of care, utilizing evidence-based criteria and clinical judgment.
- Collaborate with the treatment team to develop, implement, and evaluate individualized care plans that meet the patient's needs and optimize outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care and minimize readmissions.
- Utilize financial and insurance resources to maximize patient benefits and reduce healthcare costs.
- Identify and address clinical and utilization issues that impact patient care and outcomes.
- Provide education and support to patients and families to promote self-management and independence.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Associate's Degree in Nursing required; Bachelor's Degree in nursing or healthcare-related field preferred.
- 3+ years of relevant clinical case/care management experience in the acute care setting required.
- RN License in state of employment or compact required; Case Manager certification preferred.
The Emergency Department Care Manager will work in a fast-paced, dynamic environment that requires strong organizational and time management skills. The ideal candidate will be able to multitask, prioritize tasks, and maintain a high level of productivity in a rapidly changing environment.
